{"id":18833,"date":"2026-07-02T07:46:03","date_gmt":"2026-07-02T11:46:03","guid":{"rendered":"https:\/\/observatoireprevention.org\/?p=18833"},"modified":"2026-07-02T07:46:04","modified_gmt":"2026-07-02T11:46:04","slug":"prediabetes-remission-long-term-cardiovascular-benefits","status":"publish","type":"post","link":"https:\/\/observatoireprevention.org\/en\/2026\/07\/02\/prediabetes-remission-long-term-cardiovascular-benefits\/","title":{"rendered":"Prediabetes remission: long-term cardiovascular benefits"},"content":{"rendered":"\n<p><\/p>\n\n\n\n<p>In Canada, it is estimated that&nbsp;<a href=\"https:\/\/www.diabetes.ca\/advocacy-policies\/advocacy-reports\/national-and-provincial-backgrounders\/diabetes-in-canada?utm_source=chatgpt.com\">approximately 30% of adults<\/a>&nbsp;have dysglycemia, including prediabetes and type 2 diabetes\u2014that is, blood sugar levels higher than normal due to the development of insulin resistance, which prevents cells in the liver, muscles, and adipose tissue from effectively absorbing circulating glucose. This represents nearly 12 million people nationally, including approximately 1.6 million in Quebec. Although chronic hyperglycemia is generally not associated with visible symptoms (most affected individuals are completely unaware that they have prediabetes, and a substantial proportion have type 2 diabetes), it places a significant burden on the pancreas and considerably increases the risk of developing type 2 diabetes and its complications in the years that follow.<\/p>\n\n\n\n<p>Chronic exposure to excess blood sugar is indeed a particularly dangerous situation for cardiovascular health. Hyperglycemia creates several conditions that, collectively, significantly increase the risk of cardiovascular events: 1) chronic inflammation that damages the walls of blood vessels and creates conditions conducive to the accumulation of fatty deposits and thus the development of atherosclerosis; 2) increased blood viscosity that places mechanical stress on the vessels and the heart and promotes the development of hypertension; 3) nerve damage (neuropathy) that can affect the nerves involved in controlling heart rhythm. For all these reasons, prediabetes is associated with an increased risk of major cardiovascular events, including myocardial infarction, stroke, heart failure, and premature mortality.<\/p>\n\n\n\n<p><strong>Reversing hyperglycemia<\/strong><\/p>\n\n\n\n<p>Prediabetes is a condition that develops primarily in people with certain cardiometabolic risk factors, including excess adiposity\u2014particularly abdominal fat\u2014and low levels of physical activity.&nbsp;<a href=\"https:\/\/observatoireprevention.org\/en\/2025\/06\/19\/fat-and-sugar-the-deadly-duo-behind-insulin-resistance\/\">It is well documented<\/a>&nbsp;that fat accumulation, especially in the abdominal area, leads over time to insulin resistance and a progressive rise in blood glucose levels. These effects can be exacerbated by physical inactivity and a sedentary lifestyle due to the crucial role of physical activity and muscle mass&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S2667036423000018#:~:text=Highlights,yield%20slightly%20better%20glycemic%20benefit.\">in blood glucose control<\/a>. In practice, this influence of lifestyle on the development of prediabetes means that it is possible to&nbsp;<strong>reverse<\/strong>&nbsp;this condition and normalize blood glucose levels through lifestyle changes, particularly regarding&nbsp;<a href=\"https:\/\/www.nature.com\/articles\/s41574-020-0381-5\">diet and physical activity<\/a>&nbsp;(Figure 1). Several studies have clearly shown that these interventions are effective&nbsp;<a href=\"https:\/\/www.nejm.org\/doi\/full\/10.1056\/NEJMoa012512\">in slowing the progression of prediabetes to type 2 diabetes<\/a>&nbsp;and,&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0140673617331021?via%3Dihub\">in some cases<\/a>, can even reverse clinically diagnosed diabetes.&nbsp;&nbsp;This concept is also supported&nbsp;&nbsp;by the results of the&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S1499267122004300?via%3Dihub\">first<\/a>&nbsp;and&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S1499267125001510?via%3Dihub\">second<\/a>&nbsp;cohorts of the EPIC Centre&#8217;s Prediabetes and Diabetes Remission Clinic and the&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S2589790X24003263?via%3Dihub\">DIABEPIC-1<\/a>&nbsp;and&nbsp;<a href=\"https:\/\/dom-pubs.pericles-prod.literatumonline.com\/doi\/10.1111\/dom.70142\">DIABEPIC-2<\/a>&nbsp;studies, also conducted at the EPIC Centre.<\/p>\n\n\n\n<figure class=\"wp-block-image size-large\"><img loading=\"lazy\" decoding=\"async\" width=\"1024\" height=\"545\" src=\"https:\/\/observatoireprevention.org\/wp-content\/uploads\/2026\/07\/Fig.-1-prediabete-ENG-1024x545.jpg\" alt=\"\" class=\"wp-image-18834\" srcset=\"https:\/\/observatoireprevention.org\/wp-content\/uploads\/2026\/07\/Fig.-1-prediabete-ENG-1024x545.jpg 1024w, https:\/\/observatoireprevention.org\/wp-content\/uploads\/2026\/07\/Fig.-1-prediabete-ENG-300x160.jpg 300w, https:\/\/observatoireprevention.org\/wp-content\/uploads\/2026\/07\/Fig.-1-prediabete-ENG-768x409.jpg 768w, https:\/\/observatoireprevention.org\/wp-content\/uploads\/2026\/07\/Fig.-1-prediabete-ENG-1536x818.jpg 1536w, https:\/\/observatoireprevention.org\/wp-content\/uploads\/2026\/07\/Fig.-1-prediabete-ENG-2048x1091.jpg 2048w\" sizes=\"auto, (max-width: 1024px) 100vw, 1024px\" \/><figcaption class=\"wp-element-caption\"><strong>Figure 1. The influence of lifestyle on the development and remission of prediabetes and type 2 diabetes.\u00a0<\/strong>Genetics, lifestyle habits (such as a sedentary lifestyle), and excess adiposity can greatly influence the risk of prediabetes and its progression to type 2 diabetes (red arrows). However, numerous studies have demonstrated that these two conditions are often reversible following lifestyle interventions (diet and physical activity) (green arrows), leading to the normalization of blood glucose levels. Note that a minimum weight loss of approximately 5% of the initial weight appears to be required for prediabetes remission, while greater weight loss, on the order of 10 to 15% and sustained over time, is necessary for long-term remission of type 2 diabetes.<\/figcaption><\/figure>\n\n\n\n<p><strong>Cardiovascular benefits<\/strong><\/p>\n\n\n\n<p>A&nbsp;<a href=\"https:\/\/www.thelancet.com\/journals\/landia\/article\/PIIS2213-8587(25)00295-5\/fulltext\">recent study<\/a>&nbsp;illustrates how normalizing blood glucose levels in prediabetic individuals can not only reduce the risk of developing type 2 diabetes but also lead to long-term positive effects on cardiovascular health. In this study, researchers followed individuals who had participated in a prediabetes remission program for approximately 20 years. The program&#8217;s goal was to reduce initial body weight by at least 7% and to engage in a minimum of 150 minutes of moderate-intensity exercise per week, in line with the recommendation of 30 minutes of moderate physical activity per day, five times per week. Approximately 12% of study participants achieved these goals after one year of intervention, with an average weight loss of 7.6 kg, or about 8% of their average initial weight (92 kg, BMI = 32), and normalization of fasting blood glucose (from 5.7 to 5.2 mmol\/L). (2026).<\/a><\/figcaption><\/figure>\n\n\n\n<p>These observations therefore show 1) that it is possible to reverse prediabetes and 2) that this is associated with a major reduction in the cardiovascular complications associated with it through lifestyle modifications.<\/p>\n\n\n\n<p>These results remind us that prediabetes is not only a risk factor, but also a key period for intervention to prevent the onset of cardiometabolic complications. Normalizing blood glucose levels through sustained lifestyle changes\u2014including a healthy diet, regular physical activity, and maintaining muscle mass\u2014can permanently alter one&#8217;s health trajectory.<\/p>\n\n\n\n<p>Beyond preventing diabetes, these lifestyle habits directly contribute to protecting the heart and blood vessels for decades. In other words, every lifestyle improvement today can translate into a significantly reduced cardiovascular risk tomorrow. Screening for prediabetes, discussing it with a healthcare professional, and undertaking gradual and sustainable changes are therefore among the most powerful strategies we have to preserve long-term cardiometabolic health.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>It is known that reversing prediabetes to normal blood sugar levels significantly reduces the risk of developing diabetes later in life.
