Most studies observe a significant reduction (on the order of 25-40%) in premature mortality for exercise volumes of between 150 and 450 minutes per week of moderate activity (e.g., walking) or 75-225 minutes of vigorous activity (e.g., running).

This potential for prevention is, however, not being fully exploited: studies show that a significant proportion of the world’s population, including in North America, does not meet the minimum recommended levels of physical activity (150 minutes of moderate activity or 75 minutes of vigorous activity per week) and can therefore be considered “physically inactive.” This trend even seems to be worsening in recent years: while in 2000, approximately 23% of the world’s population was considered physically inactive, this proportion rose to 31% in 2022. In Canada, the situation is even worse, with less than half (46%) of the adult population being sufficiently active, and this percentage is likely to decrease in the coming years given the high rate of sedentary behaviour among 12- to 17-year-olds (only 30% of boys and 8% of girls are sufficiently active). There is no doubt that this high prevalence of sedentary lifestyle contributes to the incidence of several chronic diseases that affect our society; it is estimated that 6-10% of premature mortality worldwideis directly caused by the negative effects of physical inactivity on cardiovascular diseases, type 2 diabetes and certain cancers (breast and colon in particular).

A little exercise is much better than none at all!

In most surveys (see here, for example), lack of time (work, commuting, family obligations) is the most frequently cited reason for not meeting the minimum recommended physical activity (although, according to a study of 300,000 people, it seems to be more a question of priorities, as most people have at least 5 hours of free time each day on average).

Yet an often-overlooked aspect of physical activity, and one that is very encouraging for those who are less active, is that even minimal amounts of exercise can still provide significant health benefits, even when they fall below the recommended amounts. One of the best examples of these benefits comes from a large-scale study conducted in Taiwanwith approximately 500,000 people, the vast majority (75%) of whom were relatively inactive. Unsurprisingly, the study first showed that active people had a reduced risk of mortality compared to sedentary people and that the time required to enjoy these benefits was directly related to the intensity of the exercise; for example, while 20 minutes of vigorous physical activity such as jogging is associated with a 30% reduction in the risk of all-cause mortality, 80 minutes of more moderate activity such as walking (Figure 1) is required to achieve a similar level of protection.

Figure 1. Reduction in the risk of all-cause mortality according to the volume of daily physical activity. The curves represent the mortality reductions that can be achieved through daily vigorous (red) or moderate (blue) physical activity. Adapted from Wen et al. (2014).

However, a very interesting finding of the study is that it also observed a reduction in mortality risk, on the order of 10-15%, even for activity levels below the recommended thresholds (Figure 1). For very sedentary individuals, for whom reaching the recommended amount of exercise seems an unattainable goal, it is therefore entirely possible to mitigate the negative effects of a sedentary lifestyle on health and significantly improve life expectancy by engaging in a minimum amount of exercise.

A recent study confirms the benefits of very low amounts of exercise, well below recommended levels. In this study, researchers analyzed physical activity data from approximately 50,000 people living in Scandinavia and the United States. Using an accelerometer, they were able to precisely quantify the time participants spent on physical activity as well as the intensity of these exercises, specifically analyzing periods of moderate and vigorous physical activity (MVPA) (see Table 1 for examples).

Intensity

Examples

Intensity indicators

Moderate

Fast walking
Vigorous housekeeping
Landscaping maintenance (e.g., digging, lawn mowing)
Light bicycle riding

Your heart rate increases, your breathing intensifies, and you can speak (but not sing).

Vigorous

Competitive sports (e.g., tennis, soccer, etc.)
Snow removal
Jumping rope
Jogging
High-speed cycling or cycling on mountainous terrain

Your heart rate increases considerably, you breathe heavily, and you can only utter a few words without catching your breath.

Table 1. Examples of moderate and vigorous physical activity (MVPA).

The results clearly show that in inactive individuals—those who engage in less than 20 minutes of exercise per day—adding a brief 5 to10minute episode of physical activity substantially reduces the risk of mortality compared to those who remain sedentary (Figure 2). The more inactive a person is, the greater the impact of these activity episodes, illustrating how detrimental sedentary behaviour is to health and that any amount of exercise, however small, is better than none. At the population level, the authors calculated that if all insufficiently active individuals included these brief episodes of physical activity in their routine, a 10% reduction in overall premature mortality could be achieved, which is considerable.

Figure 2. In addition to reducing exercise time and depriving the body of the cardiovascular and muscular benefits of physical activity, sedentary behaviour also appears to be an independent risk factor for premature mortality due to its negative effects on blood vessel function, blood pressure, and glucose metabolism. The results show that in highly sedentary participants (10 hours or more per day), reducing sitting time by 30 minutes/day or 60 minutes/day was associated with a risk reduction of approximately 10% and 25%, respectively. The authors estimate that, at the population level, these reductions in sedentary behaviour could prevent approximately 7% of all premature mortality.

Figure 3.
